nobody has suggested to Sonny that he see his doctor since his medication is clearly not working and see if there is something else that might work. If they took blood they would find out the doses have been too low.

I don't know what Ava hopes to accomplish having Sonny go off the rails. Since she is in such close proximity she could well end up on the receiving end of his out of control condition.
